CDC Video: Building an AIDS-free Future for Over 100,000 Girls and Young Women
In Zambia, adolescent girls and young women bear a disproportionate burden of HIV infection. CDC is working to transform the outcome through the DREAMS (Determined, Resilient, Empowered, AIDS-free, Mentored, and Safe) program, which addresses the key drivers of HIV in areas with the highest rates and widest gender disparities. In just three years, over 100,000 adolescent girls and young women have been supported through CDC DREAMS. By empowering girls and young women, DREAMS is building an AIDS-free future.

Resilience in Stressful Events (RISE) is a peer responder program that supports health care workers who have emotional distress due to difficult care-related events. Developed in 2010 at The Johns Hopkins Hospital, RISE has been replicated by hospitals and health systems globally. In this episode, two of RISE’s founders — Cheryl Connors and Matt Norvell — talk about how RISE was developed, how it is run and what health care leaders should consider when creating a peer-support program.

1. A culture of connection and support is essential to improving the well-being of individuals and health care teams. Offering a peer support program, which provide a safe space to talk, is a key strategy.
2. Talking to someone who can relate and bear witness regarding the distress we face in health care can help us feel less alone and enable us to return to work and to our families without carrying the weight in silence.
3. Leaders, managers and supervisors can’t be all things to all people and need to focus on areas like safety and staffing. A peer support program, such as RISE, can impact the institution by off-loading some of the emotional support responsibility from managers.
4. Key drivers of success include: a.) a training curriculum and onboarding program for peer responders, b.) endorsement from leaders so staff feel they have permission to take time to receive support and so volunteer peer responders can help their colleagues in a rewarding way, and c.) care for the peer responders themselves. RISE offers a debriefing so volunteers can process difficult responses.

Johns Hopkins geriatrician Alicia Arbaje recaps findings from two bodies of work that address transitions of care for older adults from hospital to home health services. She describes the development of the Hospital-to-Home-Health Transition Quality (H3TQ) Index and research performed to validate the tool. The H3TQ Index measures care transition quality specific to hospital to home health transition. In testing this tool, Arbaje and colleagues found that the majority of care transitions from the hospital to the home involve safety issues that are not addressed before discharge — despite efforts to do so. The quality of care transitions still needs work, and the H3TQ Index can help identify issues that should be targeted. The plan is to make the tool available to researchers and health care systems interested in improving transitions of care quality. #H3TQ #johnshopkins
